Does Medicare Cover Home Care?

The short answer surprises most families, and it is the single most common misunderstanding in senior care.

Medicare does not pay for ongoing non-medical home care such as help with bathing, meals, housekeeping, or companionship. Medicare may cover intermittent skilled home health care, meaning nursing or therapy, when a physician certifies it is medically necessary and the person meets Medicare's criteria, including being homebound.

What Medicare home health can cover

  • Intermittent skilled nursing care
  • Physical therapy, occupational therapy, and speech-language pathology
  • Medical social services related to the treatment plan
  • Part-time home health aide services, but only while skilled care is also being provided
  • Certain medical supplies and durable medical equipment under separate rules

What Medicare does not cover

  • 24-hour care at home
  • Meal delivery
  • Homemaker services such as cleaning, laundry, and shopping when that is the only care needed
  • Personal care such as bathing and dressing when that is the only care needed
  • Long-term custodial care, which is what most families mean by home care

Private pay

The most common method. Families often combine income, savings, and contributions from multiple adult children, sometimes selling or renting a second property.

Long-term care insurance

Policies frequently cover non-medical home care, but check the elimination period, daily benefit limit, and whether the policy requires an agency rather than a private hire.

Veterans benefits

San Antonio has a very large veteran population. VA Aid and Attendance and other VA programs may help eligible veterans and surviving spouses with in-home care costs. Contact the VA or a Bexar County veterans service officer for eligibility.

Texas Medicaid programs

For those who qualify financially, Texas Medicaid waiver programs may provide attendant care services. Eligibility rules are strict and administered by the state, not by home care providers.
